Assignment 5: Context Diagram
Continuing Case: Family Health Care
Currently, when a patient enters our office, the receptionists give them a clipboard with several pages of forms to fill out. When the patient finishes filling out the forms, they give the forms back to the receptionists. The receptionists then enter the information for new patients into our patient portal system. If it is an existing patient, they check for any changes.
Family Health Care's managing partner, Dr. Stephen Nguyen, sent a letter to IT consultant, Adam Burke, to help develop the system for the new services. During the project, Adam will work closely with Maria Martinez, web master. Jayne Rogers, an intern will assist Adam. She is eager for the chance to make a good impression in your first professional job.
Background
Julie Martin has completed a preliminary investigation and performed the fact-finding tasks that were described in Chapters 2 and 4. Now, she will use the results to develop a logical model of the proposed information system.
The following information was given about patient information (names have been changed to protect privacy):
- All patients have a patient id, a family id, date of birth, and gender.
- The patient database keeps track of each appointment with date and time and which doctor was seen and the results.
- We also maintain a list of all prescriptions and supplements.
These are profiles for a few patients.
- Marie Reynolds has been a patient for 12 years. She is married to John Reynolds and they have a daughter Stacy. A baby boy is expected in 3 months. The Reynolds family lives at 35 Elm St, Rockville Md. 20895. Everyone in the family are our patients.
- Brian Gutierrez is single and lives alone. He became our patient last month when he sprained his ankle. He doesn’t take any prescription medicines, but he does take a few vitamin supplements. He has been smoking for 5 years but is trying to quit.
- Stacy Martin is a single mother with 3 children.
Tasks
Create a professional looking document to address the issues noted below. Be sure that you have corrected all spelling and grammar errors and all sections of the report are clearly labeled.
Use the information given above, and review the previous information given about the proposed system.
- Prepare a context diagram for the new system.
- Prepare a diagram 0 DFD for the new system.
- Create a Data Dictionary to show the data elements, data flows, data stores and processes for the new system using the information in the sample scenarios above. This should match the DFD. Include at least 3 specifications about the format of the data.
